Where
Dropped by the Blob crushed with the blue trap (north-west), per Berserker's FAQ and game-kouryaku. Fandom says the purple trap.
Use
Place it in the central goal (the flashing circle on the map, north side). Each shard placed opens more of the maze and spawns a new Blob that smashes a wall.
Shards needed to open the exit: Amateur 1, Normal 3, Veteran 4, Professional 4.
On Professional the map shows no Blob icons and they move faster: this is the hardest part of the episode.
